Terms of Service

1. Agreement to Terms

These Terms of Service ("Terms") govern your access to and use of the NeuroHome website at this domain and the NeuroHome telehealth program ("Services"), which are operated by NSH Operations, LLC ("NeuroHome," "we," "us," or "our"). By using the Services, you agree to be bound by these Terms. If you do not agree, please do not use the Services.

If you enroll in the NeuroHome program on behalf of a minor child, you represent that you are the child's parent or legal guardian and that you have the authority to enter into this agreement on the child's behalf.

2. Eligibility

NeuroHome is available to families in areas where we are able to deliver services. Eligibility is confirmed during the intake process.

You must be 18 years of age or older to create an account, book a Discovery call, or enroll a child in the program.

3. Nature of Services

NeuroHome is a functional-neurology and developmental support program delivered via telehealth and a clinician-prescribed home protocol. NeuroHome's Services include clinical evaluation, individualized home protocols, a clinical-grade device kit, weekly clinician-led telehealth sessions, and progress tracking using a validated standardized assessment appropriate to your child's presentation.

4. No Guarantee of Results

Every child's nervous system is different. While NeuroHome's protocols are based on more than a decade of clinical practice at NeuroSage Centers and informed by current research in functional neurology, we make no representations or warranties about specific outcomes. Testimonials and case studies presented on our website describe individual experiences and are not predictive of any particular family's results.

5. Discovery Call

The Discovery call is a free, no-commitment conversation with our team to determine whether NeuroHome is appropriate for your child. The Discovery call is not a clinical evaluation, does not establish a clinician-patient relationship, and does not constitute medical advice.

6. Enrollment, Fees, and the Home Kit

Pricing and program details are discussed during the Discovery call. Enrollment is confirmed by a separate written services agreement that sets out the specific scope, term, fees, refund policy, and clinician assignment for your family. The home kit is shipped on loan for the duration of the program and remains the property of NSH Operations, LLC; the kit must be returned in working condition at the end of the program. Damage, loss, or non-return may result in replacement charges as specified in the services agreement.

7. Cancellation and Rescheduling

Sessions may be rescheduled or cancelled through the GoHighLevel booking system with at least 24 hours' notice. Sessions cancelled within 24 hours of the scheduled time may be forfeited. Specific refund and termination terms are governed by your services agreement.

8. Telehealth Consent

By participating in NeuroHome telehealth sessions, you consent to receiving care via interactive audio and video over the Zoom for Healthcare platform. You acknowledge that telehealth has both benefits and limitations, including the possibility that technical failures may interrupt a session and that some aspects of in-person care are not available remotely. You may withdraw consent to telehealth at any time without affecting your right to seek future care.

9. Emergencies

NeuroHome is not an emergency service. If your child is experiencing a medical or psychiatric emergency (including but not limited to risk of harm to self or others, severe injury, seizure, or acute illness), call 911 or go to the nearest emergency department immediately. Do not contact NeuroHome for emergencies.

All content on this website (including text, graphics, logos, video, photographs, the NeuroSage Hub methodology, clinical protocols, and the NeuroHome brand) is the property of NSH Operations, LLC or its licensors and is protected by U.S. and international copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works without our prior written permission.

12. Privacy

Your use of the Services is also governed by our Privacy Policy. Protected Health Information generated during clinical care is governed separately by our HIPAA Notice of Privacy Practices, which you will receive at intake.

13. Disclaimers

THE SERVICES AND ALL CONTENT ARE PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS. TO THE MAXIMUM EXTENT PERMITTED BY LAW, NEUROHOME DISCLAIMS ALL WARRANTIES, WHETHER EXPRESS, IMPLIED, OR STATUTORY, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICES WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE OF HARMFUL COMPONENTS, OR THAT ANY CONTENT, INCLUDING CLINICAL CONTENT, WILL BE ACCURATE, RELIABLE, OR COMPLETE.

14. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, IN NO EVENT WILL NEUROHOME, ITS PARENT, AFFILIATES, OFFICERS, EMPLOYEES, OR CLINICIANS BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF PROFITS, REVENUE, DATA, OR GOODWILL, ARISING OUT OF OR IN CONNECTION WITH YOUR USE OF THE SERVICES, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. OUR AGGREGATE LIABILITY FOR ANY CLAIM ARISING OUT OF OR RELATING TO THESE TERMS OR THE SERVICES WILL NOT EXCEED THE AMOUNT YOU HAVE PAID TO NEUROHOME IN THE TWELVE (12) MONTHS PRECEDING THE EVENT GIVING RISE TO THE CLAIM. NOTHING IN THESE TERMS LIMITS LIABILITY THAT CANNOT BE LIMITED UNDER APPLICABLE LAW.

15. Indemnification

You agree to indemnify, defend, and hold harmless NSH Operations, LLC, its officers, employees, clinicians, and contractors from any claims, liabilities, damages, losses, and expenses (including reasonable attorneys' fees) arising out of or in any way connected with your use of the Services, your violation of these Terms, your violation of any rights of another party, or your violation of any applicable law.

16. Termination

We may suspend or terminate your access to the Services at any time, with or without cause, including for violation of these Terms or behavior that endangers staff or other families. Specific clinical termination procedures are governed by your services agreement and applicable state law. Upon termination, sections of these Terms that by their nature should survive (including disclaimers, limitation of liability, indemnification, and governing law) will survive.

17. Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of Louisiana, without regard to its conflict-of-laws principles. Any dispute arising out of or relating to these Terms or the Services will be resolved by binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, with arbitration conducted in Calcasieu Parish, Louisiana, or remotely by agreement of the parties. You and NeuroHome each waive the right to a trial by jury and the right to participate in any class action. Notwithstanding the foregoing, either party may seek injunctive or equitable relief in a court of competent jurisdiction to protect intellectual property rights.

18. Changes to These Terms

We may update these Terms from time to time. When we do, we will revise the "Last updated" date above and, where appropriate, notify enrolled families by email. Your continued use of the Services after changes constitutes acceptance of the updated Terms.
