Introduction

NeuroHome ("NeuroHome," "we," "us," or "our") is a telehealth pilot program operated by NSH Operations, LLC and led by Dr. Kyle Daigle, DC, FIBFN-CND. We respect your privacy and are committed to protecting the personally identifiable and health-related information you may provide us through this website or in the course of receiving our services.

This Privacy Policy explains what information we collect, how we use it, and under what circumstances we may disclose it. It applies to information we collect through this website and through delivery of the NeuroHome program. It does not apply to information collected from other sources.

Depending on your activities when visiting this site or enrolling in our program, you may be required to agree to additional terms and conditions, including a HIPAA Notice of Privacy Practices for clinical care.

Website Visitors

Like most website operators, NeuroHome collects non-personally-identifying information that web browsers and servers typically make available, such as browser type, language preference, referring site, and the date and time of each visitor request. Our purpose in collecting non-personally-identifying information is to better understand how visitors use this site. From time to time, we may release non-personally-identifying information in aggregate (for example, by publishing usage trends).

We may also collect potentially personally-identifying information such as Internet Protocol (IP) addresses. We disclose IP addresses only under the same circumstances we use and disclose other personally-identifying information, as described below.

Information You Provide

Certain visitors choose to interact with NeuroHome in ways that require us to collect personally-identifying information. The amount and type of information depends on the nature of the interaction. Examples include:

• Booking a Discovery call through our calendar widget (your name, email, phone, and scheduling preferences)
• Completing the Discovery Quiz (your email and self-reported responses about your child's development)
• Contacting us by email at hello@neurohome.app
• Enrolling in the NeuroHome program (clinical intake information about your child, billing details, and shipping address for the home kit)

You are not required to share any information that personally identifies you in order to browse the site. However, certain features (such as booking a call or enrolling) are not available without it.

Protected Health Information (PHI)

Once you enroll in the NeuroHome program, clinical information you share with us about your child (including intake forms, clinician notes, standardized assessment scores, and session activity) is treated as Protected Health Information under the Health Insurance Portability and Accountability Act (HIPAA). PHI is handled separately from general marketing data and is governed by our HIPAA Notice of Privacy Practices, which you will receive at intake.

Marketing-only information collected on this website (such as a Discovery Quiz email submission from someone who has not yet enrolled) is not considered PHI and is handled under this Privacy Policy.

Telehealth Sessions

NeuroHome sessions are delivered over Zoom for Healthcare, a HIPAA-eligible telehealth platform under a signed Business Associate Agreement (BAA). Sessions may be recorded only with your explicit written consent, and any recordings are stored in our HIPAA-compliant systems with the same protections as clinical notes. We do not sell, share, or use session content for marketing purposes.

How We Use Your Information

We use the information we collect to:

• Schedule and deliver your Discovery call and any subsequent clinical sessions
• Match your child with the appropriate clinician and clinical plan
• Communicate program updates, scheduling reminders, and clinical progress
• Improve our website, services, and program design
• Comply with legal and regulatory requirements

Disclosure of Information

NeuroHome discloses potentially personally-identifying and personally-identifying information only to employees, clinicians, contractors, and affiliated organizations that (i) need to know that information to process it on our behalf or to provide services available through NeuroHome, and (ii) have agreed not to disclose it to others. We do not rent or sell your information to anyone.

We may disclose your information in response to a subpoena, court order, or other governmental request, or when we believe in good faith that disclosure is reasonably necessary to protect the property or rights of NeuroHome, third parties, or the public at large.

Aggregated Statistics

We may collect statistics about visitor behavior and program outcomes. We may display these statistics publicly or share them with partners, but we do not include personally-identifying information when we do so.

Cookies

NeuroHome uses cookies and similar technologies to enrich your online experience, store your preferences, and help us understand how visitors use the site. A cookie is a small string of information stored on your computer that your browser sends back to the site on each visit.

Third-Party Services

We use third-party services to deliver NeuroHome, including GoHighLevel for scheduling, customer messaging, and marketing automation, and Zoom for Healthcare for telehealth video sessions. Both vendors operate under signed Business Associate Agreements where applicable and are bound by contracts requiring them to protect your information. We may also link to external sites we do not operate; we have no control over and assume no responsibility for those sites' privacy practices.

Security

The security of your personal information is important to us. We use commercially reasonable physical, technical, and administrative safeguards to protect your information, including encryption in transit and at rest for clinical data. However, no method of transmission over the internet or method of electronic storage is 100% secure, and we cannot guarantee absolute security.

Children's Privacy

NeuroHome's services are intended for children with parental involvement, and we collect information about minors only with the consent and through the actions of a parent or legal guardian. We do not knowingly collect personally-identifying information directly from children under 13 except through their parent or guardian. If you believe a child has provided information directly to us without parental consent, please contact us so we can remove it.

Your Choices

You can opt out of marketing communications at any time by clicking the unsubscribe link in any email we send or by contacting us at hello@neurohome.app. Opting out of marketing communications does not affect transactional or clinical communications related to active care.

You may request access to or correction of your personal information by contacting us at the email above. For Protected Health Information, please follow the process described in our HIPAA Notice of Privacy Practices.

Privacy Policy Changes

Most future changes are likely to be minor. NeuroHome may update this Privacy Policy from time to time at our sole discretion. We encourage you to check this page periodically for changes. Your continued use of the site after any change constitutes acceptance of the updated policy.
